二、蒐集個人資料之目的
依據法務部頒佈「個人資料保護法之特定目的及個人資料之類別」及國立臺中教育大學華語文中心蒐集您的個人資料之特定目的如下：040/行銷、069/契約、類似契約或其他法律關係事務、090/消費者、客戶管理與服務、109/教育或訓練行政、136/資(通)訊與資料庫管理、137/資通安全與管理、148/網路購物及其他電子商務服務、157/調查、統計與研究分析、158/學生（員）（含畢結業生）資料管理、181/其他經營合於營業登記項目或組織章程所定業務。
 

1. Purposes of Personal Data Collection

In accordance with the “Specific Purposes and Categories of Personal Data under the Personal Data Protection Act” issued by the Ministry of Justice and the specific purposes of collecting your personal data by CLC, NTCU are as follows: 040/Marketing, 069/Contracts, Similar Contracts or Other Legal Relationships Affairs, 090/Consumers, Customer Management and Service, 109/Education or Training Administration, 136/Information and Database Management, 137/Information Security and Management, 148/Online Shopping and Other E-commerce Services, 157/Surveys, Statistics, and Research Analysis, 158/Student (Staff) (Including Graduates) Data Management, 181/Other operations in accordance with business registration items or organizational articles.
 
三、蒐集個人資料之類別
依據法務部頒佈「個人資料保護法之特定目的及個人資料之類別」國立臺中教育大學華語文中心蒐集您個人資料如下。

1. Categories of Personal Data Collection

According to the “Specific Purposes and Categories of Personal Data under the Personal Data Protection Act,” CLC, NTCU collects the following personal data from you.
 

• 識別類
• C001 辨識個人者(如姓名、職稱、通訊及戶籍地址、住家電話號碼、行動電話、E-MAIL、相片及其他任何可辨識資料本人者等)。
• C002 辨識財務者(如金融機構帳戶之號碼等)。
• C003 政府資料中之辨識者(如身分證統一編號、統一證號、證照號碼、護照號碼等)。

(1) Identification Category

• C001 Personal Identifiers (such as name, title, mailing and residential addresses, home phone number, mobile phone, email, photo, and any other personally identifiable information).
• C002 Financial Identifiers (such as financial institution account numbers).
• C003 Identifiers in Government Data (such as ID number, Uniform ID Number, license number, passport number, etc.).

 
（二）特徵類

• C011 個人描述(如年齡、性別、出生年月日、國籍等)。

(2) Descriptive Category

• C011 Personal Descriptions (such as age, gender, date of birth, nationality, etc.).

 
（三）家庭情形類

• C023 家庭其他成員之細節(如子女、受扶養人、家庭其他成員或親屬、父母等)。

(3) Family Situation Category

• C023 Details of Other Family Members (such as children, dependents, other family members or relatives, parents, etc.).

 
（四）教育、考選、技術或其他專業類

• C051 學校紀錄(如大學、專科或其他學校等)。

(4) Education, Examination, Technical, or Other Professional Category

• C051 School Records (such as university, college, or other school records).

 
（五）健康與其他類

• C111 健康紀錄(如醫療報告、治療與診斷紀錄、檢驗結果等)。

(5) Health and Other Category

• C111 Health Records (such as medical reports, treatment and diagnosis records, test results, etc.).

 
四、個人資料利用之期間、地區、對象及方式
（一）期間：本校華語文中心營運期間。
（二）地區：台灣地區(包括澎湖、金門及馬祖等地區)。
（三）對象：本校、主管機關、合作廠商及學校。
（四）方式：電子文件、紙本或其他合於當時科技之適當方式。
4. Period, Region, Object, and Method of Personal Data Utilization
(1) Period: During the operation period of CLC, NTCU.
(2) Region: Taiwan (including Penghu, Kinmen, and Matsu).
(3) Object: NTCU, competent authorities, partner companies, and schools.
(4) Method: Electronic documents, paper, or other appropriate methods in line with current technology.
 
五、個資當事人得依個資法第三條規定請求查詢、閱覽、製給複製本、補充或更正、請求停止蒐集、處理或利用及請求刪除，其行使方式依法令及本中心相關規定。
5. Data Subjects may request inquiries, access, copies, supplements or corrections, request cessation of collection, processing or utilization, and request deletion in accordance with Article 3 of the Personal Data Protection Act, with the exercise methods subject to legal requirements and relevant regulations of CLC, NTCU.
 
六、個資當事人得提供資料，並應自行確認資料正確性，如未提供正確資料將不受理報名。
6. Data subjects may provide information and should confirm the accuracy of the data. Failure to provide correct information will result in the rejection of the application.
 
七、本中心保留隨時修改本聲明的權利，修改後的條款將通知傳送到您於學員資料的電子郵件地址，或於本中心網站上公告，並自公佈之日起生效。
7. CLC, NTCU reserves the right to modify this statement at any time. The modified terms will be notified to you via the email address in your student records or posted on CLC, NTCU’s website, and will be effective from the date of publication.
 
八、自我保護措施：
請您妥善保管您的密碼及或任何個人資料，勿將資料提供給他人，並於網站使用完成後務必執行登出動作。
8. Self-Protection Measures:
Please safeguard your password and any personal information. Do not provide data to others, and be sure to log out after using the website.
 
九、本聲明之解釋與適用，以及本聲明有關之爭議，均應依照中華民國法律予以處理，並以臺灣臺中地方法院為第一審管轄法院。
9. The interpretation and application of this statement, and any disputes related to this statement, shall be handled in accordance with the laws of the Republic of China, with the Taichung District Court in Taiwan as the court of first instance.
